Two factors that cause drug prices to soar were the free market procedures of three layers -- medicine factories, drug representatives, and hospital doctors; and government guided-price makers who know little about the costs of the drugs.
药价高涨原因有二。其一，药品要经药厂、医药代表、医生三次倒手才能到患者手中；其二，政府指导价制定者并不清楚药物成本。
文中的government guided-price就是指“政府指导价”，是政府对药价总体把控的最高限制价格。为了给企业薪酬标准和人们求职提供指导，有关部门还会制定income benchmark（工资指导价）。
高额的医药利润率使drug representative（医药代表）成为了一个贬义词，人们通常称之为“药代”，他们专门负责相关药品的推广。我国自去年开始实行基本药物制度，确定了一批essential medicines（基本药物）的零售价，限制了“药代”们抬高药价。同时，国家还出台政策开始试点public hospital reform（公立医院改革），旨在改进medical service system（医疗服务体系），改善doctor-patient relationship（医患关系）。
